Problem of connection apache directory studio (LDAP authentication failed)

I have installed apache directory studio, when I am trying to add a new connection from the server it is giving authentication issues.

Error:

 - Unable to connect
 - No Connection

Before last month the connection worked fine with the same credentials, but now it is not working and giving an error out of it.

How to check if the address and port which I am using are blocked on the server or not?
